5. GRANT AGREEMENT WITH AGE WELL SENIOR SERVICES FOR\nCDBG-CV FUNDING ASSISTANCE TO PURCHASE AN ADA-COMPLIANT\nVAN FOR SENIOR RESIDENT TRANSPORTATION\nRECOMMENDED ACTION: 1. Approve an Agreement with Age Well Senior\nServices, Inc. providing for an award of $33,500 of Community\nDevelopment Block Grant - Coronavirus funds to assist with the purchase\nof an ADA-compliant van for senior transportation services in response to\nthe COVID-19 pandemic and find that the Agreement is not a project\nsubject to CEQA pursuant to State CEQA Guidelines section 15378, and\neven if it is a project, then the action is exempt from CEQA pursuant to\nState CEQA Guidelines section 15061(b)(3).\n2. Authorize the Mayor to execute and the City Clerk to attest to the\nagreement with Age Well Senior Services, Inc., substantially in the form\nattached.
