36. Black Infant Health Services Sole Source Contract Amendments\nRecommendation: Approve and instruct the Director of Public Health to execute\ncontract amendments to three sole source contracts for the continued provision\nof Black Infant Health (BIH) services with The Children’s Collective, Inc. (TCC)\nfor $1,142,000, All For Kids Organization (AFK), formerly known as Children’s\nBureau of Southern California, for $904,000, and with the City of Pasadena\n(COP) for $1,004,000 (2, 4 and 5), effective July 1, 2025 through June 30,\n2026, at a total maximum obligation of $3,050,000, all fully funded by State\nGeneral Funds, Federal Title V, Assistance Listing No. (ALN) 93.994 and\nFederal Title XIX, ALN 93.778, awarded by the California Department of Public\nHealth (CDPH); and authorize the Director to take the following actions:\n(Department of Public Health) APPROVE\nExecute amendments to extend the term up to one additional year\nthrough June 30, 2027, at amounts to be determined by the Director,\nbased on funding availability, provide an increase or decrease in funding\nup to 10% above or below the annual base maximum obligation, effective\nupon execution or beginning of applicable funding period, update the\nstatement of work and/or scope of work, as necessary, and/or correct\nerrors or omissions in the contract's terms and conditions;\nExecute change notices to the contracts that authorize modifications to\nthe budget with corresponding modifications to the statement of work\nand/or scope of work, that are within the same scope of services, as\nnecessary, and changes to hours of operation and/or service locations;\nand\nImmediately suspend or terminate the contracts upon issuing a written\nnotice if contractors fail to perform and/or fully comply with contract\nrequirements, and to terminate contracts for convenience , if necessary.\n(25-2275)\nCommunity Services
