Solutions for Equitable Diabetes Prevention and Management Grant\nAward\nRecommendation: Authorize the Director of Public Health to accept and\nimplement a Grant Award in the amount of $1,000,000, with an optional cost\nsharing of $46,192 from Public Health, from the Centers for Disease Control\nand Prevention (CDC), Assistance Listing Number 93.988, for Solutions for\nEquitable Diabetes Prevention and Management, for the Year 1 budget period\nfor the period of June 30, 2023 through June 29, 2024; accept future awards\nand/or amendments that are consistent with the requirements of the CDC Grant\nthat extend the term at amounts to be determined by the CDC, and/or provide\nan increase or decrease in funding; accept future amendments that are\nconsistent with the requirements of the CDC Grant that reflect non-material\nand/or ministerial revisions to the award’s terms and conditions, and allow for\nthe rollover of unspent funds and/or redirection of funds.
