39. COVID-19 Community-Based Outreach Services Sole Source Contract\nAmendment\nRecommendation: Approve and instruct the Director of Public Health to execute\nan amendment to a sole source contract with Community Health Councils, Inc.\n(CHC) for the continued provision of COVID-19 community-based outreach\nservices, to extend the term through June 30, 2024, and increase in the amount\nby $705,635 from $30,287,392 to $30,993,027, funded by the Centers for\nDisease Control and Prevention (CDC) Epidemiology and Laboratory Capacity\nfor Prevention and Control of Emerging Infectious Diseases (ELC) funding,\nAssistance Listing Number (ALN) 93.323 and Tobacco Settlement funds; and\nauthorize the Director to take the following actions: (Department of Public\nHealth) APPROVE\nExecute an amendment to a sole source contract with Public Health\nInstitute (PHI) for the continued provision of COVID-19 community-based\noutreach services, to extend the term through March 31, 2024, and\ndecrease the amount by $1,518,937, from $15,399,028 to $13,880,091,\nfunded by CDC ELC funding, ALN 93.323, and the Partners Across\nRegions Tackling Needs for an Equitable Response Project funding, ALN\n93.391;\nExecute amendments to the contracts that allow the rollover of unspent\ncontract funds, as applicable, extend the term through December 31,\n2024, at amounts determined by the Director, based on funding\navailability, allow a no-cost extension to the term through June 30, 2025,\nprovide an increase or decrease in funding up to 10% above or below\neach term’s annual base maximum obligation, effective upon amendment\nexecution, or at the beginning of the applicable contract term, make\ncorresponding service adjustments, as necessary, and/or make revisions\nto the statement of work, as necessary;\nExecute change notices to these contracts that authorize budget\nmodifications, and corresponding service adjustments, as necessary,\nchanges to hours of operation and/or service locations, and/or\ncorrections of errors in the contract’s terms and conditions; and\nImmediately suspend the contract(s) upon issuing a written notice to\ncontractors who fail to perform and/or fully comply with program\nrequirements, terminate the contract for convenience by providing a\n30-calendar day advance written notice to the contractors, and to accept\nvoluntary contract termination notices from contractors.
