41. Fiscal Year 2023-28 Linkages Program Services Subawards\nRecommendation: Approve and authorize the Director of Aging and Disabilities\nto award and execute five Linkages Program Services Subawards with the\nfollowing Subrecipients: Human Services Association, Special Service for\nGroups, Inc., WISE & Healthy Aging, Southeast Social Services Funding\nAuthority, and Santa Clarita Valley Committee on Aging, for the term effective\nJuly 1, 2023 through June 30, 2028, and subsequently execute six\nmonth-to-month extension options, for an anticipated annual amount of\n$850,000 or a potential aggregate amount of $4,675,000, for the term of five\nyears and six months, and with allocations to the Subrecipients subject to\navailability of funding and contingent upon the Subrecipient’s performance in\nmeeting the goals of Linkages, as well as Subrecipient’s adherence to its\nSubaward terms; and authorize the Director to take the following actions:\n(Aging and Disabilities Department) APPROVE\nExecute amendments, add new, relevant, or updated Federal, State\nand/or County Subaward terms and conditions, and increase or\ndecrease the Subaward amounts that may exceed 10% of the maximum\nsubaward amount based on the availability of funding, and/or based on\nthe Subrecipient’s performance, provided that the total allocation does\nnot exceed available funding; and\nTerminate the Subawards with the Subrecipients upon their request, and\nthose that have closed or are in default of their Subaward requirement,\nfor subrecipient default or for the convenience of the County.
