37. Ending the Human Immunodeficiency Viruses Epidemic Services Contract\nAmendment\nRecommendation: Approve and authorize the Director of Public Health to\nexecute an amendment to a contract with Heluna Health for the provision of\nadditional Ending the Human Immunodeficiency Viruses Epidemic (EHE)\nservices, to increase the total maximum obligation by $18,649,209 through\nFebruary 28, 2025, 100% funded by Health Resources and Services\nAdministration EHE funds and the Centers for Disease Control and Prevention\nEHE funds; and authorize the Director to take the following actions:\n(Department of Public Health) APPROVE\nExecute amendments to the contract that extend the term through\nFebruary 28, 2027, at the revised annual base maximum obligation, allow\nfor the rollover of unspent contract funds, as allowed by the grant, and/or\nprovide an increase or decrease in funding up to 10% above or below\neach period’s revised base maximum obligation, effective upon\namendment execution or at the beginning of the applicable period, as\nnecessary;\nExecute change notices to the contract that authorize budget\nmodifications, modifications to the statement of work and/or scope of\nwork, as necessary, changes to hours of operation and/or service\nlocations, and/or corrections of errors in the contract’s terms and\nconditions; and\nImmediately suspend the contract upon issuing a written notice to the\ncontractor if it fails to perform and/or fully comply with contract\nrequirements; and terminate the contract for convenience by providing a\n30-calendar day advance written notice to the contractor.
