36. Substance Use Disorder Prevention Services Sole Source Amendment\nRecommendation: Approve and authorize the Director of Public Health to\nexecute an amendment to extend the term of a contract with San Fernando\nValley Partnership, Inc., effective January 1, 2024 through June 30, 2026, for\nthe continued provision of substance use disorder prevention services for the\nCapacity-Building Prevention Project, in the amount of $1,125,000, 100%\nfunded by Substance Abuse Prevention and Treatment Block Grant funds,\nAssistance Listing Number 93.959; and authorize the Director to take the\nfollowing actions: (Department of Public Health) APPROVE\nExecute amendments to the contract that extend the term for one\nadditional year through June 30, 2027, at an amount to be determined by\nthe Director, based on funding availability, allow the rollover of unspent\nfunds, and provide an increase or decrease in funding up to 10% above\nor below each period's base maximum obligation, effective upon\namendment execution, which may include corresponding revisions to the\nscope of work, as necessary, and make corresponding service\nadjustments, as necessary;\nExecute change notices to the contract that authorize modifications to, or\nwithin, budget categories within each budget, and corresponding service\nand funding adjustments, as necessary, changes to hours of operation\nand/or service locations, and/or corrections of errors in the Contract’s\nterms and conditions; and\nImmediately suspend the contract upon issuing five calendar days'\nadvance written notice if the contractor fails to comply with contract\nrequirements or terminate the contract for convenience by providing five\ncalendar days' advance written notice if the contractor is in substantial\nbreach of contract terms and conditions.
