36. The Provision of Data To Care Services Contracts\nRecommendation: Authorize the Director of Public Health to execute three new\ncontracts with Men’s Health Foundation, APLA Health & Wellness, and JWCH\nInstitute, Inc., selected under a competitive solicitation process, for Data to\nCare Services in the County, effective February 1, 2024 through February 28,\n2027, for a total maximum obligation of $4,625,001, 100% offset by Health\nResources and Services Administration (HRSA) Ending the HIV Epidemic: A\nPlan for America Ending the HIV Epidemic - Ryan White HIV/AIDS Program\nParts A and B funds and HRSA Ryan White Program (RWP) Part A funds; and\ntake the following actions: (Department of Public Health) APPROVE\nExecute amendments to the contracts that extend the term through\nFebruary 28, 2030, at amounts to be determined by the Director of\nPublic Health, contingent upon the availability of funds and contractor\nperformance, allow the rollover of unspent contract funds, if allowable by\nthe grantor, provide an increase or decrease in funding up to 10% above\nor below the annual base maximum obligation, update the statement of\nwork and/or scope of work, as necessary, and/or correct errors in the\ncontract's terms and conditions;\nExecute change notices to the contracts that authorize modifications to\nthe budget with corresponding modifications to the statement of work\nand/or scope of work, that are within the same scope of services, as\nnecessary; and/or changes to hours of operation and/or service\nlocations; and\nImmediately suspend or terminate any contract upon issuing a written\nnotice to contractors who fail to fully comply with program requirements,\nand terminate contracts for convenience by providing a 30-calendar day\nadvance written notice to contractors.
