46. Core HIV Care Services Sole Source Contract Amendments\nRecommendation: Authorize the Director of Public Health to execute a sole\nsource contract amendment with Special Service for Groups, Inc. (SSG), for\nthe continued provision of Core HIV Mental Health Services, to extend the term\neffective March 1, 2024 through February 28, 2025, for a total maximum\nobligation of $147,366, 100% offset by Health Resources and Services\nAdministration Ryan White Program Part A funds; and authorize the Director to\ntake the following actions: (Department of Public Health) APPROVE\nExecute amendments to the contract to extend the term through February\n28, 2026, at amounts to be determined by the Director, contingent upon\nthe availability of funds and contractor performance, allow the rollover of\nunspent contract funds, if allowable by the grantor; provide an increase\nor decrease in funding up to 25% above or below the annual base\nmaximum obligation, effective upon amendment execution, or at the\nbeginning of the applicable contract period, allow for reallocation of funds\nbetween budgets, correct errors in the contract's terms and conditions,\nand/or update the statement of work and/or scope of work, as necessary;\nExecute change notices to the contract that authorize modifications to the\nbudget with corresponding modifications to the statement of work and/or\nscope of work, that are within the same scope of services, as necessary,\nand changes to hours of operation and/or service locations; and\nImmediately suspend or terminate the contract upon issuing a written\nnotice if SSG fails to perform and/or fully comply with program\nrequirements, and terminate the contract for convenience by providing a\n30-calendar day advance written notice to SSG.
