3. Investing in the Los Angeles County CareConnect Application\nRecommendation as submitted by Supervisors Horvath and Hahn: Instruct the\nDirectors of Health Services , Mental Health, Public Health, Children and Family\nServices, and Public Social Services, the Executive Director of the Homeless\nInitiative, within the Chief Executive Office, the Chief Probation Officer, and any\nother relevant Department Heads which operate or oversee beds and/or support\nservices for vulnerable County residents, to work with the Director of Justice,\nCare and Opportunities and the Acting Chief Information Officer to do the\nfollowing:\nInclude basic information about their funded beds and service providers\nin CareConnect, including information on services provided, eligibility,\napplication and referral processes, and other core information needed to\nhelp navigate individuals to services, with these County Departments and\nagencies to collaborate with the Director of Justice, Care and\nOpportunities and the Acting Chief Information Officer to identify the key\ninformation needed, resource and staffing needs, the best technical\napproach for incorporating this information, and any programmatic and\nstrategic considerations for including this information; and\nCollaborate with the Director of Justice, Care and Opportunities and the\nActing Chief Information Officer , to develop a process to identify which\nagencies and systems are candidates to serve as connected providers in\nCareConnect, identify the applicable agencies and systems, and develop\na phased plan for establishing these service providers as connected\nproviders, including any necessary work to develop forms or other\nprocesses within CareConnect , to allow these providers to receive and\nmanage closed-loop referrals;\nInstruct the Director of Justice, Care and Opportunities, in collaboration with the\nActing Chief Information Officer and the Executive Director of the Homeless\nInitiative, within the Chief Executive Office, to explore opportunities with the\nChief Executive Officer of the Los Angeles Homeless Services Authority , for\npotential integration of Los Angeles Homeless Services Authority services and\nbeds into the CareConnect application; and\nInstruct the Director of Justice, Care and Opportunities, in collaboration with the\nDirectors of Health Services, Mental Health, and Public Health, the Executive\nDirector of the Homeless Initiative , within the Chief Executive Office, and in\ncollaboration with the Acting Chief Information Officer, to report back to the\nBoard in writing within 90 days, and quarterly thereafter, on the progress of\nmeeting the above directives, including feasibility considerations specific to the\nunique programs/services, with the progress to include the list of existing\nCounty beds and resources navigation systems, the list of County\nDepartments/agencies and their beds and service provider types , which will be\nincluded within CareConnect at launch, and ongoing progress, including any\nintegration with existing navigation systems.
