29. Healthier Communities Project Services Sole Source Contract\nAmendments\nRecommendation: Approve and authorize the Director of Public Health to\nexecute amendments to two sole source contracts to extend the terms and\nincrease the maximum obligation for the continued provision of the\nSolutions for Healthier Communities Project, effective June 30, 2024\nthrough September 29, 2028 with the University of Southern California\nSchool of Pharmacy, in the amount of $2,019,500, and the Community\nClinic Association of Los Angeles County, in the amount of $1,522,075, fully\nfunded by the Centers for Disease Control and Prevention, Solutions for\nEquitable Diabetes Prevention and Management Grant Number\nNU58DP007384 and Innovative Solutions for Cardiovascular Health Grant\nNumber NU58DP007576, Assistance Listing Number 93.988; and authorize\nthe Director to take the following actions: (Department of Public Health)\nAPPROVE\nExecute amendments to the contracts that extend the term through\nMarch 31, 2031 at amounts to be determined by the Director,\ncontingent upon the availability of funds and contractor performance,\nallow the rollover of unspent contract funds, if allowable by the\ngrantor, allow reallocation of funds between budgets, provide an\nincrease or decrease in funding up to 10% above or below the annual\nbase maximum obligation effective upon execution, update the\nstatement of work and/or scope of work, as necessary, and/or correct\nerrors in the contracts' terms and conditions;\nExecute change notices to the contracts that authorize budget\nmodifications, with corresponding modifications to the statement of\nwork and/or scope of work, that are within the same scope of\nservices, as necessary, and/or changes to hours of operation and/or\nservice locations; and\nImmediately suspend or terminate either contract upon issuing a\nwritten notice to a contractor who fails to fully comply with contractual\nrequirements, and terminate either contract for convenience by\nproviding a 30-calendar day advance written notice to the contractor.\n(24-1370)
