50. Solutions for Equitable Diabetes Prevention and Management Contract\nRecommendation: Authorize the Director of Public Health to execute a contract,\nwith Diabetes Care Partners, selected under a competitive solicitation process\nfor the provision of Diabetes Prevention and Management Services, effective\nupon date of execution through June 29, 2028, for a total maximum obligation of\n$292,500, 100% funded by Centers for Disease Control and Prevention funds,\nAssistance Listing Number 93.988; and authorize the Director to take the\nfollowing actions: (Department of Public Health) APPROVE\nExecute amendments to the contract that extend the term through\nDecember 29, 2030, at amounts determined by the Director, contingent\nupon the availability of funds and contractor performance; allow the\nrollover of unspent contract funds, if allowable by the grantor, provide an\nincrease or decrease in funding up to 10% above or below the annual\nbase maximum obligation effective upon execution, update the scope of\nwork, as necessary, and/or correct errors in the contract’s terms and\nconditions;\nExecute change notices to the contract that authorize modifications to the\nbudget with corresponding modifications to the statement of work and/or\nscope of work, that are within the same scope of services, as necessary,\nand/or changes to hours of operation and/or service locations; and\nImmediately suspend or terminate the contract upon issuing a written\nnotice to Diabetes Care Partners if it fails to fully comply with contractual\nrequirements, terminate the contract for convenience by providing a\n30-calendar day advance written notice to the contractor.
