34. Medical, Dental, Life Insurance, and Disability Plan Rates for 2025\nRecommendation: Approve the proposed premium rates for\nCounty-sponsored plans as follows: medical and dental rates for employees\nin the Choices and Options plans for the period of January 1, 2025 through\nDecember 31, 2025; medical and dental rates for employees in the Flexible\n(Flex) and MegaFlex plans for the period of January 1, 2025 through\nDecember 31, 2025; basic life and accidental death and dismemberment\ninsurance rates for represented and non-represented employees and, for\nChoices and Options employees only, optional group term life and\ndependent term life insurance rates, for the period of January 1, 2023\nthrough December 31, 2025; Optional Group Variable Universal Life and\ndependent term life insurance for Flex and MegaFlex employees for the\nperiod of January 1, 2022 through December 31, 2026; Survivor Income\nBenefit rates for MegaFlex employees for the period of January 1, 2022\nthrough December 31, 2026; and rates for Short-Term Disability,\nLong-Term Disability (LTD) and LTD Health Insurance plans, for the period\nof January 1, 2025 through December 31, 2025; and take the following\nactions:\nInstruct County Counsel to review and approve the appropriate\nagreements and/or amendments with Anthem Blue Cross and\nAnthem Blue Cross Life and Health Insurance Company (Anthem\nBlue Cross); Cigna Healthcare of California, Inc. (Cigna); Kaiser\nFoundation Health Plan, Inc. (Kaiser); Kaiser Foundation Health Plan\nof the Mid-Atlantic States, Inc. (Kaiser Mid-Atlantic);\nUnitedHealthcare of California and UnitedHealthcare Insurance\nCompany (UnitedHealthcare); Delta Dental of California (Delta Dental\nPreferred Provider Organization (PPO)); Delta Dental of California for\nDeltaCare USA (DeltaCare USA); SafeGuard Health Plans, Inc.\n(MetLife/SafeGuard); Metropolitan Life Insurance Company (MetLife);\nand Life Insurance of North America, a wholly owned subsidiary of\nNew York Life; or their successors and affiliates, as necessary, for\nthe period January 1, 2025 through December 31, 2025;\nInstruct the Chair to sign the aforementioned agreements and/or\namendments;\nApprove the proposed premium rates for the health plans sponsored\nby the Association for Los Angeles Deputy Sheriffs, Inc., the Los\nAngeles County Fire Fighters Local 1014, and the proposed premium\nrates for the California Association of Professional Employees, for the\nperiod January 1, 2025, through December 31, 2025; and\nInstruct the Auditor-Controller to make all payroll system changes\nnecessary to implement the changes recommended herein to ensure\nthat all changes in premium rates are first reflected on pay warrants\nissued on January 15, 2025.
