56. Our Social Places and Opportunities for Teens Program Evaluation\nServices Master Agreement\nRecommendation: Authorize the Director of Public Health to execute an\namendment to a Master Agreement Work Order (MAWO), with\nHarder+Company Community Research, for the provision of Our Social\nPlaces and Opportunities for Teens Program Evaluation Services, to\nincrease the maximum obligation by $34,300 from $37,200 to $71,500, for\nthe period of July 1, 2024 through June 30, 2025, 100% funded by the\nSubstance Abuse Prevention and Treatment Block Grant and Juvenile\nJustice Crime Prevention Act funds; and authorize the Director to take the\nfollowing actions: (Department of Public Health) APPROVE\nExecute amendments to the MAWO that extend the term through\nJune 30, 2027, at amounts to be determined by the Director,\ncontingent upon the availability of funds and contractor performance,\nallow for a no-cost adjustment to the term through December 31,\n2027, allow the rollover of unspent MAWO funds, if allowable by the\ngrantor, provide an increase or decrease in funding up to 95% above\nor below the annual base maximum obligation, effective upon\nexecution or beginning of applicable MAWO budget period, update\nthe statement of work and/or scope of work, as necessary, and/or\ncorrect errors in the MAWO’s terms and conditions; and\nImmediately suspend or terminate the MAWO upon issuing a written\nnotice to contractors who fail to fully comply with program\nrequirements and terminate the MAWO for convenience by providing\n30-calendar day advance written notice to contractors.
