46. Healthy Schools Los Angeles County Project Contract\nRecommendation: Authorize the Director of Public Health to execute a contract\nwith Rising Communities, selected under a competitive solicitation process, for\nthe Healthy Schools Los Angeles County Project, which will support schools in\nestablishing comprehensive programs that engage parents, guardians, staff,\nand students around various public health topics, effective upon date of\nexecution through June 30, 2027, for a total maximum obligation of $4,087,996,\n100% funded by Opioid, JUUL, and Tobacco settlement funds; and authorize\nthe Director to take the following actions: (Department of Public Health)\nAPPROVE\nExecute amendments to the contract that extend the term at amounts to\nbe determined by the Director, contingent upon the availability of funds\nand contractor performance, allow for a no-cost adjustment to the term,\nallow the rollover of unspent contract funds, if allowable by the grantor,\nallow the reallocation of funds between budgets, provide an increase or\ndecrease in funding up to 20% above or below the annual base maximum\nobligation, effective upon execution or beginning of applicable funding\nperiod, update the statement of work and/or scope of work, as\nnecessary, and/or correct errors in the contract's terms and conditions;\nExecute change notices to the contract that authorize modifications to the\nbudget with corresponding modifications to the statement of work and/or\nscope of work that are within the same scope of services, as necessary,\nand/or changes to hours of operation and/or service locations; and\nImmediately suspend or terminate the contract upon issuing a written\nnotice to the contractor if the contractor fails to fully comply with\ncontractual requirements, and terminate the contract for convenience by\nproviding a 30-calendar day advance written notice to the contractor.\n(24-4248)
