Web-Based eConsult System and Related Services Sole Source\nAgreement Amendment\nRecommendation: Authorize the Director of Health Services to execute an\namendment to a sole source agreement with Safety Net Connect, Inc. (SNC),\nfor the provision of the web-based eConsult system and related services, and to\nextend the agreement’s term for one year from October 29, 2024 through\nOctober 28, 2025, with one additional automatic one-year extension from\nOctober 29, 2025 through October 28, 2026, and increase the maximum\ncontract amount by $2,190,656, for a total of $995,328 per year for each\none-year extension, and $200,000 in new funds for additional Pool Hours,\nwhich does not include $203,402 of unspent additional Pool Hours that are\nbeing rolled over from the current term expiring on October 28, 2024, for a\nrevised additional Pool Hours amount of $403,402 for additional work during the\nextended term; and execute future amendments and/or change notices, as\napplicable to the agreement to use available additional Pool Hours to fund\nadditional work, add, delete and/or change certain terms and conditions in the\nagreement, as mandated by Federal or State law or regulation, County policy,\nand the Board, and modify the statement of work to reflect County standards\nand needs, reduce scope and add/delete contract sites.
