13.\nApprove an agreement with Hoag Clinic, dba Hoag Executive Health, in an annual average\namount of $331,150 plus annual 10% contingency for as-needed extra services, to provide an\nAnnual Fitness & Medical Examination Program for a three year term with two one-year optional\nrenewals, for a total cumulative amount not to exceed $1,655,750; and authorize the Fire Chief,\nor designees, to execute the agreement and related documents, including any cost-neutral\namendments and any documents and to take the necessary actions to implement, fund, and\nadminister the agreement.
