24. Peer Mentoring and Life Coaching Services at Rancho Successor\nForm Agreements\nRecommendations: Authorize the Director of Health Services to execute\nsuccessor form agreements, with current individual independent\ncontractors, effective January 1, 2024 through December 31, 2030, at an\nestimated total annual amount of $450,000, with funding included in the\nDepartment’s Fiscal Year 2023-24 final budget, and to be requested for\nfuture years; and authorize for the provision of Peer Mentoring and Life\nCoaching (PM/LC) services at Rancho Los Amigos National Rehabilitation\nCenter (RLANRC); and authorize the Director to take the following actions:\n(Department of Health Services) APPROVE\nExecute agreements with additional individuals who graduated from\nthe Program for PM/LC Services, to meet the needs of RLANRC,\neffective upon execution through December 31, 2030, and expand\nthe PM/LC services to other Department of Health Services hospitals\nand health centers, in the event services are needed outside of\nRLANRC;\nExecute amendments to add, delete, and/or change non-substantive\nterms and conditions as required under Federal or State law or\nregulation, County policy, Board requirement, and/or Chief Executive\nOffice requirement; execute administrative amendments, as\nnecessary, to reflect a change in duties of the Contractor and make\nnon-substantive programmatic and/or administrative adjustments;\nexecute administrative amendments to increase rates and update\npayment language as necessary; and terminate agreements pursuant\nto the termination provisions set forth in such agreements, including\nfor the convenience of the County; and\nIncrease the estimated total annual amount for all agreements by no\nmore than 15% each year, to accommodate increases in the number\nof contractors, or expansion of the contracted services to additional\nDepartment of Health Services’ hospitals and health centers.\n(23-4169)\nAttachments: Board Letter\nPublic Comment/Correspondence
