42. Home Visiting Program Services Sole Source Contract Amendment\nRecommendation: Approve and instruct the Director of Public Health to execute\nsole source a contract amendment with the Los Angeles Unified School District,\nto extend the term for the continued support of Maternal, Infant and Early\nChildhood Home Visiting Program services, effective July 1, 2023 through June\n30, 2026, in an annual amount of $936,982 and in an aggregate amount of\n$2,810,946, fully offset by Federal Health Resources and Services\nAdministration funding passed through the California Department of Public\nHealth; and authorize the Director to take the following related actions:\n(Department of Public Health) APPROVE\nExecute amendments to the contract that extend the term for up to two\nyears through June 30, 2028, at amounts to be determined by the\nDirector, based on funding availability, allow the rollover of unspent\ncontract funds, provide an increase or decrease in funding up to 10%\nabove or below each term’s annual base maximum obligation, effective\nupon amendment execution, or at the beginning of the applicable\ncontract term, make corresponding service adjustments, as necessary,\nand allow a no-cost adjustment up to six months through December 31,\n2028; and\nExecute change notices to the contract that authorize budget\nmodifications, and corresponding service adjustments, as necessary,\nchanges to hours of operation and/or service locations, and/or\ncorrections or errors in the contract’s terms and conditions.
