11.\nAN AMENDMENT TO THE DESIGN SERVICES AGREEMENT WITH COMMUNITY WORKS\nDESIGN GROUP INC., FOR ON-CALL LANDSCAPE AND ARCHITECTURAL SERVICES FOR THE\nONTARIO TOWN SQUARE GREEN ROOM AND RESTROOM IMPROVEMENTS PROJECT\nassessments, easements, reduction of retention accounts, contract amendments within\nthe authorization limits, and filing a notice of completion at the conclusion of all\nconstruction-related activities.\nThat the City Council take the following actions:\nA. Authorize the City Manager, or his designee, to execute Change Order No. 5 to the\nConstruction Contract with Optima RPM, Inc., of Irvine, California, for additional scope\nand services for the Ontario Wellness Campus Project in the amount of $158,922 for a\ntotal not-to-exceed contract amount of $5,609,648; and\nB. Authorize the City Manager, or his designee, to execute all documents required for the\ncompletion of the project which may include, but not limited to, agreements,\nassessments, easements, reduction of retention accounts, contract amendments within\nthe authorization limits, and filing a notice of completion at the conclusion of all\nconstruction-related activities.\nThat the City Council take the following actions:\nA. Authorize the City Manager, or his designee, to execute Amendment No. 4 to the\nProfessional Services Agreement with SWA Group, Inc., of Laguna Beach, California, for\nadditional architect and design services for the Grand Park Project in the amount of\n$102,000 for a total not-to-exceed contract amount of $6,702,996; and\nB. Authorize the City Manager, or his designee, to execute all documents required for the\ncompletion of the project which may include, but is not limited to agreements,\nassessments, easements, reduction of retention accounts, contracts amendments within\nthe authorization limits, and filing a notice of completion at the conclusion of all\nconstruction related activities.\nThat the City Council take the following actions:\nA. Authorize the City Manager, or his designee, to execute Amendment No. 1 to the Design\nServices Agreement with Community Works Design Group, Inc., of Riverside, California,\nto provide on-call landscape architectural services for the Ontario Town Square Green\n2.
