10. Accept the Change Order No. 4 for CDBG-Funded Street Improvements City Project No. 6599-ST\n– Rescission Notice of Completion\nThe City Council will consider: (PW)\na. Rescinding the Notice of Completion dated March 24, 2026, previously approved by the City Council on\nFebruary 12, 2026 for the (i) Citywide Residential Resurfacing Project, Phase IV, City Project No. 699-ST,\nand (ii) Citywide Sidewalk Improvement, Phase X, City Project No. 698-ST, constructed by Sequel\nContractors, Inc.; and\nb. Appropriating $758,990 in Community Development Block Grant (“CDBG”) Funds to the Residential\nResurfacing, Phase IV, City Project No. 699-ST; and\nc. Approving Contract Change Order No. 4 to Contract No. 2025-30-CC with Sequel Contractors, Inc., for\nthe construction of the Community Development Block Grant (CDBG)-funded street improvements on\nWood Avenue, Seville Avenue, Kansas Avenue, and East Frontage Road, in an amount not-to-exceed\n$879,750; and\nd. Authorizing the Assistant City Manager/Director of Public Works to execute Contract Change Order No. 4\nto Contract No. 2025-30-CC; and\ne. Authorizing the City Manager to approve up to a cumulative amount of $100,000 in contract change\norders to complete additional work that may be needed to complete construction.
