Amendment No. 4 to Agreement No. 17-1761 and Amendment No. 4 to Agreement No. 17-1762 with Tristar Risk Management, Formerly Known as Hazelrigg Claims Management Services Incorporated, a Tristar Company. Recommendation: (500) 1. Approve the Sole Source Justification for Tristar Risk Management for the continued third-party administration services for workers’ compensation and general liability claims; 2. Approve Amendment No. 4 to Agreement No. 17-1761 with Tristar Risk Management, formerly Hazelrigg Claims Management Services, Incorporated (HCMS); 3. Approve Amendment No. 4 to Agreement No. 17-1762 with Tristar Risk Management, formerly HCMS; and 4. Authorize the City Manager to execute the amendments in a form approved by the City Attorney for the Workers’ Compensation and Property and Liability claims administration services.
