21. 23-1081 Recommendation to authorize City Manager, or designee, to execute\ncontract amendments with Anthem Blue Cross, the City’s third-party\nadministrator for the Preferred Provider Organization (PPO) and the\nHealth Maintenance Organization (HMO) group health plans, and\nMedicare Supplement plan; UnitedHealthcare Medicare Advantage\nPPO Plan; Scan Health Plan for the Medicare Advantage Plan; and\nVision Service Plan (VSP) for the vision plan; CVS Caremark as the\nPrescription Benefit Manager (PBM) for the Prescription Drug Plan for\nthe PPO and HMO plans; Delta Dental for the fee-for-service dental\nplan (DPPO) and Delta Dental USA (HMO plan); The Standard\nInsurance Company for employer-paid life insurance, voluntary life\ninsurance, and short- and long-term disability insurance; Union Mutual\nLife Insurance Company (UNUM) through Larry Lambert & Associates\nInsurance Services, for long-term care insurance; and Carrum Health\nas a third-party administrator for eligible surgeries to be performed\nthrough a Centers of Excellence facility; and, any subsequent\namendments necessary to maintain current benefit levels and remain\nin compliance with state and federal laws on all plans, at the discretion\nof the City Manager.
