5. Approve agreements with Occupational Health Centers of California, dba Concentra Medical\nCenters (Concentra) and Occu-Med, Ltd. and an agreement, in substantial form, with Southern\nCalifornia Permanente Medical Group, in an amount not to exceed $600,000 for Concentra and\nan amount not to exceed $500,000 each for Occu-Med, Ltd. and Southern California Permanente\nMedical Group, to provide medical services to comply with occupational health and medical\nstandards, pre-employment medical evaluation services, and other employment medical services,\neach for a three year period, with the option to extend up to two additional years; authorize\nminimal changes that do not substantially modify the terms and conditions of the agreement with\nSouthern California Permanente Medical Group, provided the changes are determined to be de\nminimis by the City Attorney’s Office; and authorize the Human Resources Director, or designee,\nto execute the agreements, renewal options, and related documents and to take the necessary\nactions to implement and administer the agreements.
