(32) 25-1135 CD 4 MOTION (RAMAN - YAROSLAVSKY) relative to amending the prior Council Action of October 7, 2025, (C.F. : 25-1135) relative to the Medical Care and Social Services in Council District 4. Recommendation for Council action, SUBJECT TO THE APPROVAL OF THE MAYOR: AMEND the Council Action of October 7, 2025, (Council file No. 25- 1135) relative to the Medical Care and Social Services in Council District Four, to extend the contract term for street medicine services in Council District 4 to August 31, 2027 and to amend the total amount to $1,357,990, as outlined below: 1. APPROVE and APPROPRIATE $1,190,511.52 from the General City Purposes (GCP) Additional Homeless Services Fund No. 100/56, Account No. 000618 (Additional Homeless Services - CD 4), to City Administrative Officer (CAO) Fund No. 100/10, Account No. 003040, for the street medicine contract with Healthcare In Action in Council District 4, which provides medical care and social services for people experiencing homelessness in Council District 4 through August 31, 2027. 2. AUTHORIZE the CAO to prepare Controller instructions or make necessary technical adjustments, including to the names of the Special Fund account recommended for this report, to implement the intent of these transactions; and, REQUEST the Controller to implement these instructions.
