53. Laboratory and Clinic Equipment and Medical Device Maintenance,\nCalibration, and Repair Services Sole Source Contract Amendment\nRecommendation: Approve and authorize the Director of Public Health to\nexecute a sole source amendment to a contract with Superior Scientific, Inc.,\nfor the continued provision of laboratory and clinic equipment and medical\ndevice maintenance, calibration, and repair services, to extend the term through\nDecember 31, 2024, in the amount of $64,105, 100% funded by existing\nDepartmental resources; and authorize the Director to take the following\nactions: (Department of Public Health) APPROVE\nExecute amendments to the contract that extend the term for an\nadditional six months through June 30, 2025, in the amount of $64,105,\ncontingent upon the availability of funds and contractor performance,\nallow the rollover of unspent contract funds, provide an increase or\ndecrease in funding up to 10% above or below the annual base maximum\ncontract amount, add, replace, or delete laboratory and clinic equipment\nand medical devices, update the statement of work, as necessary, and/or\ncorrect errors in the contract’s terms and conditions;\nExecute change notices to the contract that authorize revisions to the list\nof equipment and/or medical devices that do not change the maximum\nobligation, and changes to the frequency, hours, and/or service locations\nof scheduled services; and\nImmediately suspend or terminate the contract upon issuing a written\nnotice if Superior Scientific, Inc. fails to perform and/or fully comply with\ncontract requirements, and terminate the contract for convenience by\nproviding a 30-calendar day advance written notice to the contractor, if\nnecessary.
