31. Community Engagement and Related Services to Support the Healthy\nBrain Initiative Master Agreement\nRecommendation: Authorize the Director of Public Health to execute a\ncompetitively solicited Master Agreement Work Order (MAWO), with Los\nAngeles County - University of Southern California Medical Center Foundation,\nInc., for the provision of community engagement and related services to support\nthe Department of Public Health’s Division of Chronic Disease and Injury\nPrevention Healthy Brain Initiative: Los Angeles, effective upon execution\nthrough June 30, 2025, at a total maximum obligation not to exceed $597,000,\nfunded by the California Department of Public Health; and authorize the\nDirector to take the following actions: (Department of Public Health)\nAPPROVE\nExecute future amendments to the MAWO that extend the term for two\nadditional one-year periods through June 30, 2027, at amounts to be\ndetermined by the Director, contingent upon the availability of funds and\ncontractor performance, allow the rollover of unspent MAWO funds,\nand/or provide an increase or decrease in funding up to 10% above or\nbelow the annual base maximum obligation, effective upon amendment\nexecution, update the statement of work and/or scope of work, as\nnecessary, and/or correct errors in the MAWO’s terms and conditions;\nExecute change notices to the MAWO that authorize modifications to the\nbudget with corresponding modifications to the statement of and/or scope\nof work, that are within the same scope of services, as necessary,\nand/or changes to hours of operation and/or service locations; and\nImmediately suspend or terminate the MAWO upon issuing a written\nnotice to the contractor if the contractor fails to fully comply with\nrequirements of the MAWO, or terminate the MAWO for convenience by\nproviding a 30-calendar day advance written notice to the contractor.\n(24-2803)
