C. Adopt a Resolution Adopting a New Job Description and 21-2382\nSalary Range for the Emergency Services Coordinator\nClassification\nAdopt Resolution No. 2023-087, a resolution of the City Council of the City\nof Fontana adopting a new job description and salary range for the\nEmergency Services Coordinator classification.

D. Final Acceptance of Street and Sewer Improvements on 21-2392\nBanana Avenue for Tract No. 18135\n1. Accept as complete the street and sewer improvements on Banana\nAvenue for Tract No. 18135 located south of Foothill Boulevard and east\nof Banana Avenue.\n2. Adopt Resolution No. 2023-089, accepting sewer in Banana Avenue for\nTract No. 18135 as part of the City sewer system.

E. Business License Compliance Program 21-2406\nAuthorize the one-time waiver of penalties for businesses or individuals who\nare brought into compliance with City Code Chapter 15-36 through the\nBusiness License Compliance Program.

F. Acceptance of Final Map for Parcel Map No. 20585 and 21-2409\nAbandonment of a Certain Portion of Right of Way of\nBanana Avenue\nAccept Parcel Map No. 20585 located at the northeast corner of Banana\nAvenue and Durian Avenue; accept as public dedicated streets and\neasements; and authorize the Mayor and City Clerk to certify the\nabandonment of a certain portion of right of way of Banana Avenue.

G. Award a Construction Contract for the Blanchard Avenue 21-2417\nSidewalk Gap Project\n1. Award bid and authorize the City Manager to execute a construction\ncontract with SNZ Contractors, Inc. for the construction of the Blanchard\nAve Sidewalk Gap Project in the amount of $115,095.00 and authorize a\n10% contingency in the amount of $11,509.50 (Bid No. SB-126-DE-23).\n2. Appropriate $102,000 of Air Quality Management District (AQMD) Fund\n241 to the Blanchard Avenue Sidewalk Gap Project No. 3115.\n3. Approve and authorize the City Manager to execute any and all utility\nagreements, utility easements, and subsequent agreements on behalf of\nthe City of Fontana for the Blanchard Ave Sidewalk Gap Project.

H. Approve a Contract with TA Mentoring Services LLC for the 21-2419\nNoble Youth Program to be delivered at Fontana Expanded\nLearning Program school sites.\n1. Approve a contract with TA Mentoring Services LLC to provide and deliver\nthe Noble Youth Leadership Program at the Fontana Expanded Learning\nProgram’s twenty-eight (28) Elementary School sites and the five (5)\nMiddle School sites.\n2. Approve the Non-Competitive Proposal purchase under the Purchasing\nPolicies 3.1.5 for Single Source/Sole Source method.\n3. Authorize the City Manager or designee to sign the agreement, all related\ndocuments, and any amendments for the term of the agreement.

I. Approve a Contract EBK & Company LLC to provide and 21-2420\ndeliver the US Engineering League for the Fontana\nExpanded Learning Program.\n1. Approve a contract with EBK & Company LLC to provide and deliver the\nUS Engineering League, Mechanical Engineering/Robotics Club for the\nFontana Expanded Learning Program at ten (10) Elementary School sites\nand the five (5) Middle School sites.\n2. Approve the Non-Competitive Proposal purchase under the Purchasing\nPolicies 3.1.5 for Single Source/Sole Source method.\n3. Authorize the City Manager or designee to sign the agreement, all related\ndocuments, and any amendments for the term of the agreement.

J. Approve the purchase of Sandy Spin Inc. Skillastics Speed 21-2421\nStacks curriculum to be delivered at Fontana Expanded\nLearning Program’s school sites.\n1. Approve purchase of Sandy Spin Inc. Skillastics Speed Stacks\nCurriculum, an additional component to the current physical education\ncurriculum at all sites of the Fontana Expanded Learning Program.\n2. Approve the Non-Competitive Proposal purchase under the Purchasing\nPolicies 3.1.5 for Single Source/Sole Source method.\n3. Authorize the Purchasing Division to issue a purchase order for the\nSpeed Stack Skillastics Custom Package curriculum from Sandy Spin Slade.

K. Approve the purchase ZTag curriculum to be delivered at 21-2422\nFontana Expanded Learning Program’s school sites.\n1. Approve purchase of ZTag, an additional component to support academic\nand physical education development of participants at all sites of the\nFontana Expanded Learning Program.\n2. Approve the Non-Competitive Proposal purchase under the Purchasing\nPolicies 3.1.5 for Single Source/Sole Source method.\n3. Authorize the Purchasing Division is issue a purchase order for the ZUES\nv2 curriculum from ZTag.

L. Approve an Encroachment Agreement to Construct and 21-2423\nMaintain Improvements Beneath the Etiwanda Avenue\nOverpass\nApprove and authorize the City Manager to execute an Encroachment\nAgreement between the City of Fontana and Union Pacific Railroad\nCompany to construct and maintain improvements located beneath the\nEtiwanda Avenue overpass at the intersection of Etiwanda Avenue and North\nLoop Road

M. Award bid for the Active Transportation Program (ATP) 21-2424\nSafe Routes to School (SRTS) Pedestrian Improvements\nProject on Ramona, Alder, Locust Avenue Project [Federal\nID No. ATPSB1L-5307(027)] (Bid No. SB-67-DE-23).\n1. Award bid and authorize the City Manager to execute a construction\ncontract with Riverside Construction Company, Inc. for the construction of\nthe Active Transportation Program (ATP) Safe Routes to School (SRTS)\nPedestrian Improvements Project along Ramona, Alder, Locust Avenue,\nFederal ID No. ATPSB1L-5307(027), in the amount of $4,218,475.00 and\nauthorize a 10% contingency in the amount of $421,847.50 (Bid No. SB-\n67-DE-23).\n2. Approve and authorize the City Manager to execute any and all utility\nagreements, utility easements, and subsequent agreements on behalf of\nthe City of Fontana for the Active Transportation Program (ATP) Safe\nRoutes to School (SRTS) Pedestrian Improvements Project on Ramona,\nAlder, Locust Avenue Project Federal ID No. ATPSB1L-5307(027) (Bid\nNo. SB-67-DE-23).\n3. N. Award a Contract for the Demolition of Slovene Hall. 21-2425\nAward bid and Authorize the City Manager to execute a contract with Interior\nDemolition in the amount of $128,168.00 for the Demolition of Slovene Hall\nand authorize a 10% contingency in the amount of $12,816.00.

O. Award a Construction Contract for the Arrow Boulevard at 21-2426\nCypress Avenue Traffic Signal Project (Bid No.\nSB-162-DE-23).\n1. Award bid and authorize the City Manager to execute a construction\ncontract with DBX, Inc. for the construction of the Arrow Boulevard at\nCypress Avenue Traffic Signal Project (Bid No. SB-162-DE-23) in the\namount of $1,628,816.00 and authorize a 10% contingency in the amount\nof $162,881.60 (Bid No. SB-67-DE-23).\n2. Approve and authorize the City Manager to execute any and all utility\nagreements, utility easements, and subsequent agreements on behalf of\nthe City of Fontana for the Arrow Boulevard at Cypress Avenue Traffic\nSignal Project (Bid No. SB-162-DE-23).\n3. Appropriate $945,000.00 from Local Measure I Fund No. 246 to the Arrow\nBoulevard at Cypress Avenue Traffic Signal Project No. 3386.

P. Award bid (SB-112-CS23) to Carreras Tours LLC., to 21-2427\nprovide bussing transportation for the Fontana Expanded\nLearning Program.\n1. Award bid (SB-112-CS-23) to Carreras Tours LLC. for Transportation\nServices for the Fontana Expanded Learning Program (FELP) for the\nFiscal Year 2023-24.\n2. Authorize the Purchasing Division to issue a purchase order to\nCarreras Tours for the FELP transportation service for Fiscal Year\n2023-24.

Q. Approve a Task Order for Engineering Design Services for 21-2430\nthe Courtplace Housing Development Project\n(Infrastructure Improvements)\n1. Approve and authorize the City Manager to execute Task Order No. 18 in\na not-to-exceed amount of $266,446.00 with TKE Engineering, Inc. for\nEngineering Design Services for the Courtplace Housing Development\nProject (Infrastructure Improvements) Request for Proposals SQ-87-DE-\n19-58.\n2. Approve and authorize the City Manager to execute any future\namendments to the Task Order.\n3. Authorize the Purchase of a Altec AT37G Aerial Truck. 21-2434\nAuthorize the Purchasing Division to use the Sourcewell Cooperative\nPurchasing Agreement (Contract #110421-ALT) for the purchase of one (1)\n2024 Ford F-550 AT37G aerial truck for $152,203.00, plus sales tax (7.75%)\n$11,796.00, CA tire fee $136.00, and a delivery fee of $7,102.00 for a total of\n$171,237.00.

U. Approve Crossing Guard Service Agreement between 21-2439\nFontana Unified School District, Etiwanda School District,\nand the City of Fontana\n1. Approve Amendment No. 2 of the Crossing Guard Services Agreement\nwith Fontana Unified School District (FUSD), Etiwanda School District\n(ESD), and the City of Fontana (COF) for a one-year (1) term renewing\nyear three (3) of the contract at the new hourly rate of $29.50.\n2. Approve a recurring appropriation from General Fund #101 to\n10140114.8130 to provide for the annual contract increase over the prior\nyear in the amount of $10,440.00.

V. Award a Construction Contract for the S. Highland Avenue 21-2441\nat Mango Avenue Traffic Signal Project (Bid No.\nSB-97-DE-23).\n1. Award bid and authorize the City Manager to execute a construction\ncontract with Alfaro Communications Construction, Inc. for the\nconstruction of S. Highland Avenue at Mango Avenue Traffic Signal\nProject in the amount of $823,929.64 and authorize a 10%\ncontingency in the amount of $82,392.96 (Bid No. SB-97-DE-23).\n2. Approve and authorize the City Manager to execute any and all utility\nagreements, utility easements, and subsequent agreements on behalf\nof the City of Fontana for the S. Highland Avenue at Mango Avenue\nTraffic Signal Project.

W. Award a Construction Contract for the Active 21-2442\nTransportation Program (ATP) Fontana Safe Routes to\nSchool (SRTS) Gap Closure Project [Federal ID No.\nATPSB1L-5307(029)] (Bid No. DE-24-SB-01).\n1. Award bid and authorize the City Manager to execute a construction\ncontract with CT&T Concrete Paving, Inc. for the construction of the\nActive Transportation Program (ATP) Fontana Safe Routes to School\n(SRTS) Gap Closure Project Federal ID No. ATPSB1L-5307(029), in the\namount of $3,615,087.50 and authorize a 10% contingency in the amount\nof $361,508.75 (Bid No. DE-24-SB-01).\n2. Approve and authorize the City Manager to execute any and all utility\nagreements, utility easements, and subsequent agreements on behalf of\nthe City of Fontana for the Active Transportation Program (ATP) Fontana\nSafe Routes to School (SRTS) Gap Closure Project, Federal ID No.\nATPSB1L-5307(029), (Bid No. DE-24-SB-01).\n3. Appropriate $735,000.00 from Local Measure I Fund No. 246 to the\nFontana Safe Routes to School (SRTS) Gap Closure Project No. 0003.

X. Award a Construction Contract for the Alder Middle School 21-2443\nSafe Routes to School (SRTS) Project on (Bid No.\nSB-146-DE-23).\n1. Award bid and authorize the City Manager to execute a construction\ncontract with All American Asphalt for the construction of the Alder Middle\nSchool Safe Routes to School (SRTS) Project in the amount of\n$2,639,021.00 and authorize a 10% contingency in the amount of\n$263,902.10 (Bid No. SB-146-DE-23).\n2. Approve and authorize the City Manager to execute any and all utility\nagreements, utility easements, and subsequent agreements on behalf of\nthe City of Fontana for the Alder Middle School Safe Routes to School\n(SRTS) Project (Bid No. SB-146-DE-23).\n3. Appropriate $1,875,783.00 from Local Measure I Fund No. 246 to the\nAlder Middle School Safe Routes to School (SRTS) Project No. 3367.

Y. Declaring Surplus Land - Property located at 14355 Foothill 21-2450\nBoulevard\n1. Approve Resolution No. 2023-096, declaring that the City-owned\nproperty on Foothill Blvd, east of Almond Avenue (Assessor’s Parcel\nNumber 0230-051-03) is “Surplus Land” pursuant to the Surplus Land\nAct, Government Code Section 54220 Et Seq.; and approving the form of\nNotice of Availability.\n2. Authorize the City Manager and/or his designee(s) to take all actions\nnecessary or appropriate to comply with the Surplus Land Act.

Z. Award a Contract for the interior renovation for Stage RED 21-2452\n1. Approve and authorize the City Manager to award and execute a contract\nwith the lowest responsible/responsive bidder for the renovation of Stage\nRED (Bid No. DE-24-25-SB) in an amount not to exceed $825,000.\n2. Authorize the City Manager to execute any and all contract amendments\non behalf of the City of Fontana for the renovation of Stage RED.

A. Public Hearing for Consolidated Annual Performance 21-2396\nEvaluation Report (CAPER)\n1. Conduct a public hearing for the Consolidated Annual Performance and\nEvaluation Report prepared for the U.S. Department of Housing & Urban\nDevelopment to report on specific Federal housing assistance and\ncommunity development activities undertaken by the City of Fontana\nduring Fiscal Year 2022-2023; and\n2. Authorize the City Manager to execute and transmit any documents\nnecessary to submit the Consolidated Annual Performance, along with\nany comments received during the public hearing and public comment\nperiod, to the Department of Housing & Urban Development.
